ASHTABULA, Ohio (AP) — State records show an Ohio nursing home that had a fire caused by a methamphetamine lab had been cited for 18 violations last year.

A 31-year-old man died after the fire Sunday night at Park Haven Home in Ashtabula, east of Cleveland.

The home's alleged violations in a December survey included not providing adequate care, failing to investigate how a resident was injured and not properly responding to residents' complaints about missing property. Two earlier 2011 surveys found a total of two violations.

The facility did not respond to a message Tuesday.

Six more people were injured Sunday, and four of them were hospitalized. Updates on their conditions were not available Tuesday.

Police said charges were expected against two men who were burned.
